Home Telemonitoring Program
A home telemonitor is a small device placed in your home,
which is programmed to record your vital signs daily: weight, blood pressure, heart
rate, oxygen saturation and temperature. A few yes or no questions will
also be asked, for example, “Are your legs swollen more than usual?” or “Are you
having more difficulty breathing compared to a normal day?” The vital signs and
questions are transmitted to our office and reviewed by a nurse everyday. By
evaluating your vital signs, weight and the answers to the questions we can
detect changes in your condition and
evaluate your status seven days a week.
Changes are communicated to your doctor. A supervisor will call you in between
actual home visits to review your readings, check how you are feeling and give
you instruction over the phone if need be. This program helps our patients identify
early signs of medical problems and address these problems before they lead to
preventable visits to the emergency room or admission to the hospital. The telemonitoring is absolutely free to our home care patients
so you will not be
charged for this great service.
