Skilled Nursing- Professional registered nurses assess the home situation and plan a program to assure quality comprehensive care from admission to discharge. Their goal is to develop a plan of care which will assist patients and their families in resuming and maintaining an optimum level of functioning and independence. Nurses use various clinical pathways in developing plans for certain frequently treated diagnoses in order to achieve patient-specific goals. Licensed Practical Nurses- are paired up as team members functioning under the registered nurse to assist in implementing the plan of care. Homemaker-Home Health Aides- State and National certified paraprofessionals provide personal care and assistance with activities of daily living.
Physical Therapy- Professional therapists provide gait training, star training, balance, stretching, progressive ambulation, range of motion, and manual therapy techniques to assist patients in achieving maximum physical goals. Chest physical therapy and cardiac rehabilitation are also available. Occupational Therapy- Professional therapists provide services to increase a patient's ability to perform daily living tasks such as meal preparation, dressing and personal care, evaluate and establish wheelchair fabrication. Speech Language Pathology- Professional therapists provide for diagnosis and treatment of speech, language, cognitive and swallowing disorders with the goal of improving communication techniques. Medical Social Services- Assist patients and families in the process of adapting to the many changes that may occur, either functionally or emotionally, due to illness or disability including end-of-life-support. |
